Article 1 of this arrêté imposes the entry conditions (including the visa requirement) on every foreigner entering Martinique who does not benefit from free movement, and Article 3 plus Annexe II create the visa exemptions. It is the instrument that creates the obligation, not a description of it; the DGEF page only describes the same rules.

The Annexe II entries for Monaco and San Marino extend the visa exemption to stays exceeding 90 days, so no stay cap from this instrument applies to their passport holders.
