Republican Decree-Law No. 47 of 1991 creates the entry obligation itself: Article 3 provides that no foreigner may enter the Republic without a valid passport and an entry visa endorsed on it, and Article 4 delegates any nationality exemption to a decision of the Minister of the Interior. The embassy page and the eVisa portal describe or implement that obligation; they do not create it.
Article 3 of Law 47/1991 makes an endorsed entry visa a condition of entering the Republic for every foreigner ('no foreigner may enter ... unless ... an entry visa is endorsed on the passport'); these issuers appear on no exemption or visa-on-arrival list I hold, so the visa requirement applies to them and must be arranged in advance.
